A personal mission for dignified care
FORYOH Choice Home Care was founded with a personal mission: to ensure that seniors and individuals with disabilities in Northern Virginia receive the same standard of compassionate, dignified care that our founder would want for their own family.
With a background rooted in healthcare and community service, founder and CEO Sheku Foryoh recognized a growing need for home care providers who combine clinical reliability with genuine warmth. That vision led to the founding of FORYOH Choice Home Care, built on a simple belief: every client deserves to age, or recover, in place, safely and with dignity.
Under this leadership, FORYOH Choice Home Care has built a team of trained, background-checked caregivers and established rigorous quality assurance, training, and compliance standards designed to meet and exceed Virginia Department of Health licensing requirements.

Licensing & Regulatory Compliance
FORYOH Choice Home Care is a Virginia-registered LLC operating in compliance with applicable Virginia Department of Health (VDH) home care organization standards, with Home Care Organization licensure in progress. Our compliance framework includes:
- Compliance with the Code of Virginia licensure requirements for Home Care Organizations
- Adherence to 12VAC5-381 (Rules and Regulations for the Licensure of Home Care Organizations)
- Maintenance of required policies, procedures, and client/employee records for inspection
- Participation in required VDH surveys and inspections
Caregiver Screening & Training
Every caregiver completes a rigorous onboarding program before their first client visit, including criminal background and registry checks, employment-eligibility and reference verification, TB screening and health clearance, and CPR/First Aid certification. Initial training covers personal care techniques and body mechanics, infection control, HIPAA and confidentiality, emergency procedures, dementia care fundamentals, and client rights. Ongoing continuing education meets Virginia licensing requirements with specialized modules and supervised competency checks.
Quality Assurance Program
Our structured Quality Assurance & Performance Improvement (QAPI) program ensures consistent, safe, and effective care. Supervisory home visits occur within the first two weeks of service and at least every 60 to 90 days thereafter, with random and scheduled quality checks, client and family satisfaction surveys, and regular review of caregiver documentation. Findings from incident reports, complaints, and surveys are reviewed in monthly QA meetings to drive continuous improvement.
HIPAA & Confidentiality
We protect the privacy and confidentiality of all client health information in full compliance with HIPAA and applicable Virginia privacy laws. Access to Protected Health Information is limited to staff directly involved in a client's care on a need-to-know basis, all employees sign confidentiality agreements and complete HIPAA training, and client information is never disclosed to third parties without written authorization except as required by law. A complete Notice of Privacy Practices is provided to every client at the start of service.
Emergency Preparedness
We maintain a written Emergency Preparedness Plan addressing natural disasters, severe weather, power outages, and public health emergencies. Each client has an individualized emergency plan on file, clients are triaged by acuity to prioritize check-ins during declared emergencies, and a 24/7 on-call coordinator with backup staffing protocols ensures continuity of care.
